High load with paddles, you are probably just pussyfooting the shifter. Its a very common problem on many bikes/quads.

Possibly a bent shift fork, but usually when that happens the engagement dog does not full engages and it slips out of sencond. Very common on bikes, because the shift hits the ground when you dump the bike.
